Register for NAFA:
There are 2 registration options

1. Download the NAFA registration form (pdf). From your computer, you can fill in all applicable fields. Once complete, simply click the Submit button at the bottom of the form. Doing so will launch your email client with the completed registration form attached. The email will already be addressed to the appropriate NAFA personnel. Feel free to add any comments/questions to the message body and then send the email.
2. Download the NAFA registration form (pdf). From your computer, you can fill in all applicable fields. Print the completed form and return it to the church office.

Please note that if you are not satisfied with your responses, you may click the Reset button at the bottom of the form. Doing so will clear all form fields.
