Registering for 24/7 is easy with our online registration form. Simply fill in the form below and click "Submit Registration Form ". Please make sure that all information is correct prior to submitting forms. Only one form is required per family. Form fields with and asterisk (*) are required. Please accompany children to the check in desk on the first night as a signature from a parent or legal guardian will be required to complete the registration. You will be required to indicate that you have read and agree with the Terms and Conditions of this registration form prior to submitting your information.

We do most of our contact through e-mail.
Both the student and parent will receive periodic e-mails on 24/7 activities & events.

Terms & Conditions

  1. I understand that my child(ren) may participate in physical activities such as those held during Game Time. As with any physical activity, there is a risk of injury. I fully accept this risk and hold harmless from any legal liability, Northside Baptist Church and any persons involved with the AWANA Ministry.
  2. In the event of any emergency that requires medical treatment for the above child(ren), I understand every effort will be made to contact me or my emergency contact. However, if I/we cannot be reached, I give my permission to the AWANA volunteers to secure the services of a licensed phycisian to provide the care necessary for my child's well-being. I assume responsibility for all costs connected to any accident or treatment of my child(ren).
  3. I grant permission for photos of my child to be taken for in-church use only.
  4. I grant permission for my child's leader/director/commander to contact my child by mail, e-mail or phone.
  5. I grant permission to travel to/from AWANA events with an adult leader. Any such even will be clearly communicated with me beforehand.
  6. Additional Release forms may be necessary for specific events/trips. Form(s) will be provided when necessary.
